چکیده

Tests for growth hormone secretion SIR,-We were interested in the correspondence between Dr Addy and Professor Brook and Dr Hindmarsh on detecting organic illness through assessment of height velocity .' 2 The question, however, is academic and their arguments theoretical since, as we have recently shown, the assessment of height velocity in individuals is so imprecise as to be clinically meaningless. There is effectively no correlation between successive velocities, so that a 12 month velocity cannot possibly identify anyone.3'4 The Wessex Growth Study provides a practical demonstration of the problem. Thirty (17%) of the original cohort of 174 school entrants below the third height centile had identifiable organic disease. Eighteen of these were monitored over a period of three years alongside a cohort of 78 short 'normal' children in whom all pathology had been excluded. The height velocity of four of the short 'normal' children and two of those with organic disease was deemed to be on or below the third centile for velocity after the first year. Over the next 12 months, two of the short 'normal' subjects, and three of those with organic disease, were growing at a rate below the third centile, but the identity of the 'poor growers' changed from year one to year two. We should abandon the notion that low 12 month velocities will effectively identify pathology. The children in the Wessex Growth Study with organic causes of short stature were identified at the beginning of year one on the basis of height screening alone.

Background and purpose : Growth hormone deficiency is the reason of growth disorder in children. Therapeutic response is related to the factors such as, age, drug dosage, numbers of administrations and duration of therapy. Researches have been done to clarify the relationship between therapeutic response with the maximum amount of GH secretion in stimulatory tests. Ïn order to study this r...

Growth and Development in a incidentally diagnosed Resistance to Thyroid Hormone

Resistance to thyroid hormone is an uncommon condition. We report the clinical and laboratory findings of a case with both resistance to thyroid hormone and, a reduced capacity to produce and respond to thyroid hormone. RTH is a disorder characterized by elevated circulating thyroid hormones, state of non-suppressed pituitary TSH secretion and refractoriness to hormone action in peripheral tiss...

Hazards of pharmacological tests of growth hormone secretion.

Children with growth hormone deficiency can now be treated with biosynthetic human growth hormone. Before treatment is started the pituitary function of the child should be investigated. This is usually done with a pharmacological test of growth hormone secretion,' the commonest test being insulin induced hypo-glycaemia. The results of these tests may, however, be difficult to interpret,I espec...

An assessment of growth hormone provocation tests.

There are a number of tests available for assessment of anterior pituitary hormone function. Recent concern has centred on the use of provocative tests for growth hormone release and in particular the glucagon and insulin induced hypoglycaemia or insulin tolerance test (ITT). These tests have been associated with cerebral oedema and death of at least two children and, as a result, questions hav...

Familial growth hormone releasing factor deficiency in pseudopseudohypoparathyroidism.

A mother with pseudopseudohypoparathyroidism and her short son showed poor spontaneous growth hormone secretion, and provocation tests suggested a deficiency of growth hormone releasing factor. This is the first report of growth hormone releasing factor deficiency in pseudopseudohypoparathyroidism. The boy has responded well to growth hormone treatment over a period of three years.

Effect of lipids on growth hormone secretion in humans.

To determine the effect of elevations of plasma lipids on growth hormone secretion in humans, paired insulin hypoglycemia tests and paired arginine infusion tests were performed on eight and six normal female volunteers respectively. On 1 of the 2 test days for each growth hormone stimulus, subjects were given 60 g corn oil (Lipomul) 3 hr before testing followed by intravenous heparin (5000 U) ...
